Glucose Homeostasis and Neuroendocrine Regulation in Critical Illness

چکیده
By definition, critical illness is any condition requiring support for failing vital functions, either with mechanical aids or with pharmacological agents (1). Concept of the modern critical care firstly appeared during the poliomyelitis epidemic of 1952 in Denmark. The increasing hospital admissions of patients with respiratory failure brought about the need for a distinct medical staff and department specifically managing these critically ill patients. Beyond that epidemic, critical care was accepted and

Critical illness-induced dysglycaemia: diabetes and beyond
Type 2 diabetes has reached epidemic proportions in many parts of the world. The disease is projected to continue to increase and double within the foreseeable future. Dysglycaemia develops in the form of hyperglycaemia, hypoglycaemia and marked glucose variability in critically ill adults whether they are known to have premorbid diabetes or not. Patients with such glucose dysregulation have in...
متن کاملPOMC neurons expressing leptin receptors coordinate metabolic responses to fasting via suppression of leptin levels
Leptin is critical for energy balance, glucose homeostasis, and for metabolic and neuroendocrine adaptations to starvation. A prevalent model predicts that leptin's actions are mediated through pro-opiomelanocortin (POMC) neurons that express leptin receptors (LEPRs). However, previous studies have used prenatal genetic manipulations, which may be subject to developmental compensation. Here, we...
متن کاملThe endocrine response to critical illness: update and implications for emergency medicine.
This review will provide an updated overview of the neuroendocrine response to critical illness. Specifically, the current evidence for "stress steroid" administration will be examined, as well as interventional glucose control during critical illness. The emergency physician will also find relevance in the alterations of thyroid hormones that occur in the face of severe illness or trauma.
متن کاملFibroblast growth factor 21 response to critical illness: effect of blood glucose control and relation with mitochondrial dysfunction, the integrated stress response and survival
Introduction Fibroblast growth factor 21 (FGF21) is a recently identified hormone that regulates metabolic homeostasis, with a potential role as a stress hormone [1]. Critical illness is hallmarked by insulin resistance and mitochondrial damage, metabolic alterations that increase circulating levels of FGF21 via the integrated stress response (ISR) [2]. Targeting normoglycemia during critical i...
متن کاملRole of Neuronal Glucosensing in the Regulation of Energy Homeostasis
Glucosensing is a property of specialized neurons in the brain that regulate their membrane potential and firing rate as a function of ambient glucose levels. These neurons have several similarities to and -cells in the pancreas, which are also responsive to ambient glucose levels. Many use glucokinase as a rate-limiting step in the production of ATP and its effects on membrane potential and io...
